PLEASE READ - IMPORTANT INFORMATION
Effective December 20, the Ontario government will begin requiring proof of vaccination for youth aged 12 to 17 years of age participating in organized sports at recreational facilities. Proof of ID to verify name and date of birth will also be required. Photo ID is not required. Please make sure to send any players over the age of 12 with ID (a photo of the original ID should be sufficient) and proof of vaccine to any practices or games after December 20. Proof will be required every time the player enters the arena. 

Examples of identification that may be use include:

  • birth certificate
  • driver’s license
  • government (Ontario or other) issued identification card - Health card is valid ID
  • passport
  • citizenship card
  • permanent resident (PR) card
  • Indian Status Card or Indigenous Membership Card
